To be admitted into the Social Services Program, clients will first go through a brief intake assessment with required documents to verify eligibility into the program.


Intake process involves all required documents below:

Age Verification

  • 55+ or older
  • 18-54 with disabilities

*** Provide Identification Card/Driver's License ***

Income Verification

  • Past 3 months of current pay stubs
  • Unemployment benefits letter
  • Social Security letter/award

Housing Verification

  • Utility bill (gas, water, electric)
  • Mortgage/rental agreement

***Must have current name and address on utility bill and housing agreement***
